We have 2 convenient locations to serve our patients:

Conroe, TX - 500 Medical Center Blvd, Suite 218, Conroe, Texas 77304
Dr. Elzaim will be seeing patients at the Conroe location Monday, Thursday, & Friday 8am until 5pm. Tuesday 8am until noon.

Cleveland, TX - 203 North College Avenue, Suite #3001, Cleveland, Texas 77327
Dr. Elzaim will be seeing patients at the Cleveland location Tuesday from 1pm until 5pm & Wednesday 8am until 5pm.
